In Italian, Momenti means moments.
Thank you to all who joined us on May 31 at Bella Sera!
We celebrated our moments – moments of compassion, of innovation, of dedication. Moments that have shaped Presbyterian SeniorCare Network over the past 90 years.
Recipients of this award are distinguished for their exceptional service in support of aging-related concerns and for making a significant, positive impact on the lives of older and disabled persons. This year, we honor Carla Falkenstein who worked with our Presbyterian SeniorCare Network team through her career at Pennsylvania Housing Finance Agency. Recently retired, Carla was a champion for the Service Coordination and Nursing Home Transitions programs across Pennsylvania, and her support of the Service Enriched Housing concept has led to the success of these programs across the state.
With this award, we honor extraordinary contributions in enhancing the quality of life of our residents. We salute Washington Health System for its partnership throughout our more than 50-year history in Washington County. The hospital has supported Presbyterian SeniorCare Network initiatives to enhance the quality of medical services and manage chronic diseases.
This award is presented for outstanding contributions of time, service or support. Since 2008, Your Holiday Home Committee members have lent their considerable talents and time to present each year an outstanding fundraising event to support the residents of Presbyterian SeniorCare Network in Washington. The event has grown each year, raising nearly $200,000 for the Washington campus.
This new, state-of-the-art residential care community is designed specifically for the growing population of people with Alzheimer’s and related dementias in the Washington County region and beyond. It will build upon the 25-year legacy of Woodside Place of Oakmont, recognized for its pioneering and uniquely person-centered care model.LEARN MORE
